The Accelerating Change Transformation Team (ACTT) helps Alberta physicians and practice teams implement evidence-based practices to enhance the care of their patients. In alignment with Alberta’s Primary Health Care Strategy, ACTT strives to cultivate a culture of quality improvement to support physicians and teams with the work of building the best medical homes for Albertans.
We work with PCN, non-PCN and community-based specialist teams to implement specific, measurable, practical and evidence-based changes. Through our programs, services and tools TOP is helping meet the challenge of keeping practices current in an environment of progressive change. We are dedicated to quality improvement within our health system and believe together we can continue to do great things to our health system.
In 2016 the support programs of Practice Management Program, Towards Optimized Practice, and PCN Program Management Office began work together to better integrate the support and change services offered. These programs officially merged on November 1, 2018 and were renamed Accelerating Change Transformation Team or ACTT.

In the Patient's Medical Home, family physicians lead coordinated and continuous patient care by working collaboratively with other health care providers in community, specialty and acute care.
The Health Neighbourhood includes the key services which together represent the health journey. It includes the Patient's Medical Home and other health and social care services such as specialists, hospitals, laboratory, emergency medical services, home care, and continuing care.
