Team-Based Care

Overview​

Teams in a high functioning patient's medical home exhibit higher levels of satisfaction, experience less burnout, and achieve a higher quality of care. (Can we back this up with the infographic) (Medical homes work toward maintaining healthy team culture, which results in highly effective and efficient teams, and leads to increased job satisfaction and happier, healthier patients.) Primary care teams need to invest in their own development so that they possess the skills and abilities to meet patient care needs in a comprehensive and coordinated manner. 

In the PMH model, care is provided by team members working in the clinic and interdisciplinary healthcare professionals (e.g., nurses, pharmacists, mental health consultants, kinesiologists, dietitians, etc.) who may be co-located or deliver care for paneled patients in another location. In a PMH, the team has shifted from a 'referral' approach to a collaborative shared approach in providing optimal patient-centred care.
