The posters were developed with recommendations made in the IMAGINE Citizens report, input from patients registered with AlbertaPatients.ca and partners Alberta Health, Alberta Health Services and the Alberta Medical Association.
The message in the posters reflects what patients care about, in language they understand, represented by diverse images of real people. Use of all the posters is encouraged, however some may more accurately portray a specific panel of patients.
The evidence shows that continuity has many benefits. It also supports care coordination, team-based care and patient-centeredness which are foundational elements of the Patient's Medical Home.
