Engaging the Team

Introduction:

There is an opioid crisis in Alberta and primary care is part of the solution. The patient's medical home team will need to be engaged to support process improvements for patients taking opioids. The tools and resources below may be used to engage physicians, multidisciplinary team members, patients, and Primary Care Network stakeholders in this important work.

Objectives:

  1. Recognize that there is an opioid crisis in Alberta and that primary care is part of the solution.
  2. Describe how opioid process improvements align with the implementation of the Patient's Medical Home.
  3. Create rationale for utilizing change packages to provide a step-wise approach to Patient's Medical Home transformation.
