The Patient's Medical Home is not a one-size-fits-all implementation approach - i.e. each practice team’s transformational journey will be different. To support practice teams with the work of becoming medical homes for their patients, Primary Care Networks will need to invest in both quality improvement and facilitation capacity to support their members in making meaningful, sustainable and continuous improvements
In addition to supports for healthcare teams to work together to improve patient access, efficiency and clinical care, Alberta AlM offers facilitation development sessions and advanced training for quality improvement theory and application through their Art and Science of Innovation and Improvement workshops.
